Our Diabetes Program was created to enhance the education and support offered to our patients suffering with diabetes. We are wholeheartedly focused on improving the lives of our patients with diabetes. Our Diabetes Program goals are to:
- reduce the occurrence of serious illness that could result in hospital stays and ER visits
- reduce diabetes-related complications, such as blindness, amputations, kidney failure and heart disease
- provide state-of-the-art treatment, monitoring and education
- promote self-management and a team approach to diabetes care
- reduce your health care costs
- and, most important to us: make your life with diabetes better and longer
Patient Care
Patients in the diabetes program work closely and meet regularly with our diabetes educator. We have three available options for patients to receive education. Please click on any of the following options for additional details:
