Overall Results:
To qualify as a Diabetes Physician Recognition Program (DPRP), Clay Platte Family Medicine Clinic was required to conduct a readiness evaluation prior to applying for their DPRP status. Clay Platte Family Medicine Clinic randomly selected 25 diabetic patients per physician between the ages of 18 and 75. The overall score for this readiness survey sample was a perfect 100 out of a possible 100 points. Since this score does exceed the 75 point threshold, this sample could be submitted to obtain recognition for NCQA’s DPRP (see Table 2).

CONCLUSIONS
The results of this analysis demonstrate that Clay-Platte Family Medicine Clinic has met all criteria needed for NCQA’s DPRP recognition and was subsequently granted the recognition in April 2008.
